Data Subject Access Request (DSAR)
In accordance with the General Data Protection Regulation (GDPR), individuals have the right to request their data and any supplementary information held by Modality LLP; this is commonly known as a Data Subject Access Request (DSAR). This includes electronic records we hold for patients, as well as any digital recordings, such as CCTV and telephone call recordings.
Requests can be made verbally, or in writing and can come from the individual themselves or from a third party on behalf of the individual (such as relative, friend, police, solicitors), by completing a subject access request form. In order to release information, we will require the request form, along with signed consent and ID from the individual requesting the information. This also applies to third party requests.

Where individuals require copies of investigation results (blood tests/radiology), we will facilitate this without using the full DSAR procedure. The individual can submit a copy of their ID to the service email in order for us to release the information.
All requests are processed free of charge unless they are either complex, repetitive, or unfounded.
